Job Description

Clinical Pharmacist will ensure the rational and effective use of medication, dispenses medications and prescriptions in accordance with accepted standards and established Pharmacy procedures, patient counseling, monitor patient drug therapies, and provide drug information. To implement national & international standards KEY RESPONSIBILITIES: Prepares and dispenses medications. Contacts prescribers regarding unclear or problem medication orders. Reconciles patient profiles and medication administration records. Transcribes orders into computerized patient medication profiles. Review drug therapy regimens for contraindications, therapeutic duplications, drug- drug interactions, drug–food interactions, drug–disease interactions, drug–laboratory interactions, allergies, and appropriateness of drug and dose. Promotes the appropriate, safe, and effective use of antibiotic by reviewing individual patient's medical records and medication regimens for adverse drug reactions, drug interactions, appropriate indications, contraindications, drug duplications, pharmacokinetically sound dosing, and optimal drug selection
